ABSTRACT

Objective:Continuous glucose monitoring (CGM) technology is used to compare the advantages of insulin degludec (IDeg) as a basal insulin regimen compared with insulin glargine (IGlar) in the treatment of adult type 1 diabetes mellitus.Methods:30 adult patients with T1DM admitted to Heji Hospital Affiliated to Changzhi Medical College from September 2019 to December 2020 were screened. According to the random number table method, the patients were randomly divided into two groups (insulin degludec group and insulin glargine group) at a ratio of 1∶1, respectively treated with IDeg, IGlar and aspartate insulin for 12 weeks. The main outcome measures were the coefficient of variation of blood glucose (CV), mean amplitude of glycemic excursions (MAGE), time in range (TIR), time above range (TAR) and time below range (TBR). The secondary outcome measures were mean blood glucose (MBG), standard deviation of blood glucose (SD), fasting blood glucose (FPG), 2 h postprandial blood glucose (2 h BG), hemoglobin A1c (HbA 1c), means of daily differences (MOOD), and the frequency of hypoglycemic events. Results:At 12 weeks of treatment, the HbA 1c, FPG, 2 h BG, MBG, SD, CV and MAGE of insulin degludec group were lower than those of insulin glargine group, with statistically significant difference (all P<0.05). The TIR in the insulin degludec group was significantly higher than that in the insulin glargine group [73(63, 75)% vs 43(28, 63)%, P<0.001], and the TAR was lower than that in the glycerine group [25(17, 23)% vs 35(33, 64)%, P=0.003]. From the curve spectrum of blood glucose level of the two groups, the stability of blood glucose in the insulin degludec group was better than that in the insulin glargine group. After 12 weeks of treatment, 8 cases (8/15) in insulin degludec group had HbA 1c<7.0%, and 4 cases (4/15) in insulin glargine group had HbA 1c<7.0%, without statistically significant difference ( P=0.264). There were 7 cases (7/15) in the insulin degludec group and 1 case (1/15) in the insulin glargine group who achieved high quality blood glucose control, with statistically significant difference ( P=0.035). At the 12th week of outpatient follow-up, the incidence of nocturnal hypoglycemic events in insulin degludec group was significantly lower than that in insulin glargine group (4/15 vs 11/15, P=0.027). Conclusions:Compared with insulin glargine, insulin degludec can achieve higher blood glucose compliance rate, lower blood glucose level and reduce blood glucose fluctuations in patients with type 1 diabetes.

ABSTRACT

Objective:To explore the incidence of vascular leakage after acute hemodilution in patients with traumatic orthopedics by using 6% hydroxyethyl starch 130/0.4 (HES).Methods:Using prospective cohort study method, 48 orthopedic trauma patients in in Yantaishan Hospital from June 2018 to December 2018 were selected as the subjects of observation. The American Society of anesthesiologists (ASA) grade was divided into grade I-III.According to the degree of trauma, they were divided into two groups: general orthopedic patients group (24 cases) and severe trauma orthopedic patients group (24 cases). According to the formula of blood volume, the blood volume of the patients in the two groups was calculated. After intubation, 10% of the blood volume of HES was infused intravenously at the rate of 0.5 ml/(kg·min) for acute hemodilution.Plasma colloidal osmolality and hemoglobin were measured immediately before acute hemodilution (T0), 15 minutes (T1) and 30 minutes (T2) after acute hemodilution. The concentrations of HES in T1 and T2 plasma were measured. The urine volume from the beginning of infusion to 30 minutes after the end of infusion was saved. The urine volume and hes concentration were measured to calculate the urine hes content. Results:The amount of HES input was the same in the general orthopedic patients group and the severe trauma orthopedic patients group, which were (7.71±0.3) ml/kg and (7.70±0.2) ml/kg, and the expansion ratio was about 100%.Compared with T0, plasma colloid osmotic pressure at T1 and T2 were (27.9±1.5) mmHg(1 mmHg=0.133 kPa)) and (27.7±1.5) mmHg in the general traumatic orthopedics patients, which was higher than T0((26.5±1.5) mmHg, P<0.05). There was no significant difference of COP at T1 and T2 ((27.0±1.6) mmHg and(26.9±1.5) mmHg) compared with T0((26.3±1.7) mmHg, P>0.05) in the severe trauma orthopedic patients). The concentration of plasma HES in the severe trauma orthopedic patients ((6.8±0.6) g/L and (5.8±0.5) g/L) was lower than in the general traumatic orthopedics patients ((7.7±0.5) g/L and (7.1±0.5) g/L, t=5.660 and 6.755, all P<0.05) at T1 and T2.There was no significant difference of the urine HES content ((29.0±3.5 ) mg vs.(28.4±3.3) mg, t=0.61, P>0.05 )between the two groups after infusion. Conclusion:The ratio of acute hemodilution and volume expansion of HES was the same in the two groups. The changes of plasma colloid osmotic pressure and HES concentration were lower in patients with severe trauma orthopedics, and there was more obvious extravascular leakage in patients with severe trauma orthopedics.

ABSTRACT

Objective:To investigate the value of SFRP1 gene promoter methylation in PTC tissues, to eanalyze its correlation with clinical characteristics and biochemical indicators and its effect on mRNA expression, and to explore its value in clinical applications.Methods:97 patients with first-onset PTC were selected as the research subjects, and the patients’ PTC tissues (observation group) and adjacent tissues (control group) were taken. Methylation-specific PCR (MSR) was used to detect the methylation status of the SFRP1 gene promoter region, and mRNA expression was measured by fluorescent quantitative PCR. The methyltransferase inhibitor 5-Azacytidine was used to treat thyroid papillary cancer cell lines, detection of biochemical indicators and collection of clinical characteristics. Results:The positive rate of methylation of SFRP1 gene promoter region (70.1%) in PTC tissues was significantly higher than that of adjacent tissues (33.0%) ( χ2=26.747, P<0.001) ; the expression of SFRP1 gene mRNA in PTC tissues was significant MRNA expression in tissues adjacent to cancer ( t=2.886, P=0.007) , mRNA expression in methylated positive PTC tissues was also significantly lower than that in methylated negative PTC tissues ( t=2.065, P=0.038) ; with the concentration of 5-Azacytidine increased, the expression of SFRP1 gene mRNA in CGTHW-3 cells also gradually increased. In addition, the positive methylation rate in PTC tissues was related to the TNM stage ( χ2=5.487, P=0.019) and whether there was lymph node metastasis ( χ2=4.659, P=0.031) . The levels of TSH and TGAb in the methylation group were higher than those in the unmethylated group (TSH: t=2.234, P=0.023; TGAb: t=2.312, P=0.021) . Conclusion:High DNA methylation in the promoter region of SFRP1 gene can participate in the occurrence and development of PTC by down-regulating the mRNA expression of genes.
